Fixing a mistake is one thing. Making sure it never happens again is another, and it is the harder of the two.
Most professionals, after something goes wrong, resolve to be more careful. They mean it. And yet the same kinds of errors recur across careers, across teams, across the whole profession, not because people are careless but because good intentions are a weak defence against the way human beings actually make mistakes. This course is about building something stronger than good intentions.
It teaches you why errors repeat, how to find the specific conditions that make you personally more likely to slip, and how to design safeguards that hold even on your worst day. This is the difference between hoping you will not repeat a mistake and engineering it so that you cannot easily do so.
By the end of this course you will be able to
- Explain why human error recurs, and why willpower alone does not prevent it
- Distinguish the different types of error and what each one needs
- Identify the personal and situational triggers that raise your own risk
- Design safeguards using the hierarchy of controls, from strongest to weakest
- Build a prevention plan that turns each trigger into a concrete safeguard
- Monitor your safeguards so they keep working long after the original concern
How this course is different from remediation
If you have looked at the Remediation course in this series, it is worth being clear about the difference. Remediation is about addressing a specific concern that has been raised and evidencing that you have changed. This course is about the forward-looking system that stops the concern from recurring in the first place. Remediation looks back and fixes. Prevention looks forward and protects. They complement each other, but they are not the same skill, and this course teaches the one the others do not.
Who this course is for
This course is written for all UK registered healthcare professionals across the eight regulators. It is valuable if you have made a mistake and want to be sure it will not recur, if a regulator or employer has asked you to show how you will prevent repetition, or simply if you want to make your own practice more reliable before anything ever goes wrong. Prevention is a skill every professional benefits from, not only those facing a concern.
